NHS Booster Club Funding Request Form
  • All forms must be submitted by the Head Coach at least one week prior to the next Boosters Meeting. 
  • Requests should be vetted by the AD prior to submission. 
  • Coaches SHOULD NOT pay for items/services and request reimbursement. All purchases and/or subsequent reimbursement requests should be made by an associated "Friends of Needham" or Boosters Club Representative.
  • Requests should benefit the entire program when possible, not just the Varsity Team.
  • All requests are heard at the Boosters Club Meetings (generally the first Monday of each month at 7pm in the Cafeteria). Captains or team representatives must be present to speak on behalf of the program.
