OHS NEW VOLUNTEER APPLICATION - MUST BE COMPLETED BY PARENT OR ADULT 18 YEARS+ (see available positions at https://ohlonehumanesociety.org/volunteer/ . 

OHS DOES NOT HAVE THE CAPACITY TO SUPPORT PROGRAMS FOR IMMIGRATION VISAS, OPTIONAL PRACTICAL TRAINING, OR COURT-ORDERED SERVICE. ALL VIRTUAL VOLUNTEERS MUST RESIDE IN THE UNITED STATES.

YOUTH SERVICE  for one time events or done in a day ONLY: PARENTS MUST PROVIDE PERMISSION BY COMPLETING  Onsite Volunteer / One Event or Project Waiver 2025 if you or your student will only be volunteering ONE TIME! Volunteering more than one time will require this application below or parents will need to complete a one time waiver each time a student participates in an event. We cannot process student service without parental consent. 
PLEASE NOTE: 
  • ONSITE volunteers must BE ABLE TO TRAVEL to FREMONT, NEWARK OR UNION CITY for volunteer shifts.
  • YOU MUST HAVE YOUR OWN PET TO WORK WITH PET THERAPY.
  • Virtual positions may be possible from any location depending on program manager preference and availability.
  • Volunteers must be 18 years old to be trained at the Wildlife Rehabilitation Center (WRC).

By completing and submitting this agreement I understand I am providing permission for my child to participate in OHS activities and that my answers I submit indicate I am legally and financially responsible for my child's volunteer services with OHS.

OHS GENERAL VOLUNTEER AGREEMENT & RELEASE
I am over 18 years old and have voluntarily agreed to serve as a volunteer for OHS, a nonprofit corporation organized under the laws of the State of California.
OHS has permission to use photographs of me (and my child named above) in materials that promote OHS or to publicize any event, without compensation or notification.
I agree to complete all required orientation, training, and documentation relating to my volunteer position(s).
I agree to disclose any physical or psychological limitations to the supervising OHS volunteer staff before participating in any activity. 
I understand I may be interacting with healthy and unhealthy animals, and may be lifting, carrying, moving, or transporting animals if assigned. I will inform the volunteer coordinator immediately of any physical limitations I may have prior to engaging in physical tasks.
I agree In the interest of the safety of the animals, staff, and volunteers that OHS has the right to revoke my volunteer privileges if these rules and regulations are not followed and that either I or OHS can terminate this relationship at will.
I agree to provide my services without compensation and understand that OHS volunteers are not classified as employees and are ineligible for Workers’ Compensation.
I agree to be legally and financially responsible and will indemnify and hold OHS harmless for my own acts and omissions relating to the service(s) I am voluntarily providing to OHS. I am responsible for providing my own medical, liability, and auto insurance while volunteering for OHS.
I understand I am voluntarily participating in the activities of OHS with full knowledge of the risks involved and agree to accept any and all risks including but not limited to that of injury, death, or damage to myself and/or my personal property as a result of my volunteer duties, jobs, assignments, and other related tasks.
In consideration for being permitted by OHS to participate in activities and provide Services, I hereby agree that I, and/or my legal representatives will not make a claim against, sue, or attach the property of OHS for injury or damage resulting from any act, omission, negligence or other acts, caused, by any employee, agent, contractor, or representative of OHS as a result of my participation in activities and performance of the services. I release OHS from all actions, claims, or demands that I, and/or my legal representatives now have or may hereafter have for injury or damage resulting from my participation in activities and performance of the services.
​I acknowledge that this agreement will apply to the entire term of my volunteer relationship and continuing as long as I continue to be a volunteer and thereafter as is necessary to protect the interests, confidentiality, and rights of OHS with respect to use of my work products without compensation.
By submitting this agreement, I admit that I have read, understood and agreed to the terms stated herein. I fully understand that I will relinquish all claims or actions known now or in the future against OHS and that I am of legal age and legally competent to sign this agreement. I am signing this agreement of my own free will without the influence of an OHS staff member.

SOME PROGRAMS WILL REQUIRE YOU TO DRIVE ANIMALS FROM OHS LOCATIONS TO OHS EVENTS OR TO VETERINARY APPOINTMENTS.  INSURANCE REQUIRES THAT IF YOU INTEND TO DRIVE YOUR PERSONAL VEHICLE EVEN ONCE TO PERFORM ASSIGNED DUTIES FOR OHS, YOU MUST SUBMIT DOCUMENTS  PLEASE READ AND COMPLETE REQUIREMENTS BELOW.  Thank you.

All employees or volunteers using their personal vehicle must meet the following qualifications:
❖ Must have a valid California Driver’s License
❖ May not have any major moving vehicle violations or accidents (including, but not limited to DUI, Vehicular Homicide, Suspension or revocation of license) in the past 3years.
Must have a current personal auto insurance policy with a minimum of $100,000/$300,000 liability

In addition to the above, drivers who transport animals must adhere to these guidelines
❖ Animals will be kept no closer than the rear seat of the vehicle.
❖ No animal is to be transported in the open bed of any trucks.
❖ Proper temperature and ventilation will be maintained in the vehicle at all times that the animal is in the vehicle  (including when the vehicle is not being operated)

Upon hire or start of volunteer service you will be requested to email documents to your program manager: 
Copy of your current drivers’ license  
❖ Copy of your personal auto insurance policy declaration page and expiration date 
❖ Order and provide a copy of your DMV driver's record

BEFORE SUBMITTING THIS APPLICATION, YOU WILL NEED TO ACCESS AND DOWNLOAD OUR VOLUNTEER HANDBOOK and our OHS GENERAL VOLUNTEER ORIENTATION TRAINING and pass the quiz BEFORE YOUR PROGRAM MANAGER CAN DISCUSS OPTIONS WITH YOU and provide additional program-specific training. In the orientation booklet, you will find the contact for your program of interest. You can take the quiz as many times as you like and once you pass and you receive the certificate in your email (check spam), please contact your program manager.
