Anthem Community Council Membership Information Update
This form is brought to you by the Anthem Community Council. The Community Center and Civic Building are transitioning to a new recreation/membership software system! All residents with an existing membership will need to update their (and their family's) information. This form must be completed by February 26, 2018 in order for you to have access to the Community Center and all Anthem programs/activities when the new software launches in March. If an updated form is not received by February 26, 2018, there will be delays when you enter the Community Center or Civic Building.

Hard copies of this form are available at the Community Center and Civic Building front desks. When completed, please submit hard copies to Membership Personnel at the Community Center, or Welcome Center Staff at the Civic Building. All information provided will be verified upon submission.

Once the new system is live, your membership photo and green key tag will need to be updated. This can be done at the Community Center or Civic Building.
