理学系研究科等　新型コロナウイルス感染者（5名以上）報告フォーム（R5年5月8日~）/the COVID-19 Group Infection Report Form
【報告対象となる集団感染】
学生・教職員が同時期に同活動単位内で、従来の目安である５名以上の罹患者が集団で発生し、教育・研究・大学運営活動に影響が大きいと考えられる事案が発生した場合は、以下のフォームに入力（報告）してください。なお、報告された情報は、理学系および本部新型コロナウイルス対策タスクフォース内のみの取り扱いとします。
If a group of 5 or more, which is the conventional guideline of an outbreak, students, faculty or staff of the same activity group are infected at the same time, and is considered that it will have a significant impact on the education, research or operation of the university, please complete the form below. The information obtained will only be handled by the School of Science /the UTokyo Novel Coronavirus Task Force.
