Once submitted, a team member will follow up to confirm your cancellation.

Please note: Cancellations are not retroactive. We do not issue refunds or credits for dates prior to this form’s submission. We require at least 10 days notice before your next billing date to process cancellations. Your youth/teen will continue to have access through the end of their last paid billing cycle.
