This form has been created for families of children on IEPs and 504s to share concerns and feedback with the NSPAC team. Concerns will be logged and shared internally with NSPAC board members and with members of the district administration. Please note, information will be communicated to the district as provided. Please only include identifying information if you are comfortable with it being shared. All identifying information questions (grade, school, district, contact information) are strictly optional.
