UH Hilo will be sponsoring a vaccination clinic on campus on Thursday, August 19, 2021 from 12:00pm-5:00pm at the Hale Kehau residence hall lounge. This is a closed clinic and vaccinations will be available only for UH Hilo students.

If you have not yet been vaccinated and would like to participate in this clinic, please complete the short form below.

You must bring a valid picture ID with you. If you have insurance, please bring your insurance card. If you do not have insurance or an insurance card, you will still be able to receive the vaccine.

As a reminder, on August 5, the University of Hawaiʻi reminded students coming on to campuses and UH facilities, including residence halls, that COVID-19 vaccination has been added to the student health clearance requirements for Fall 2021. Students who are not fully vaccinated will have to undergo weekly COVID-19 testing. You can visit http://go.hawaii.edu/g9P for more details.
