1. Print out and complete Transportation Waiver/Medical Authorization Form (https://drive.google.com/file/d/0B_dYrJpydjtCTXZIckJIZHB5NmM/view?usp=sharing)
2. Turn in a $70 check made out to "MVHS" for team dues with the completed Transportation Waiver/Medical Authorization Form IN AN ENVELOPE WITH YOUR NAME ON IT to Mr. Long (the science department chair). Forms with check can be left on Mr. Long's desk in E120 or given directly to him.
3. Fill in all information below.
Registration is due by Thursday, March 15th. Practice begins the first week of April at Indian Plains High School and will be every Monday, Tuesday, and Thursday from 2:30 - 4:30pm.
Go to https://play.usaultimate.org/members/login/ to join. There is a $36 registration fee for youth players.
Returning players must renew their membership by paying their USA Ultimate dues and re-signing the online waiver.
