Withdrawing from Foothills Middle School
Dear Foothills Parents,
If your child/children will not be attending Foothills Middle School for the 2021-2022 school year because you will be moving or transferring to another school out of the district, we ask that you please complete the form below to withdraw your child from school. We would like to assist families in expediting the withdrawal process. This form will serve as your official notification to Foothills that your child will be attending another school for the 2021- 2022 school year.

If your child is currently an 8th grade student and will be attending a school other than Arcadia High School in the fall, we ask for you to also complete the form below. All other 8th grade students will automatically be enrolled at Arcadia High School for the 2021-2022 school year.
