Cloverleaf Pride Award Nomination Form
The Cloverleaf Pride award has been established to recognize individuals who have provided exemplary service to the Cloverleaf Local School district.  The award was created to acknowledge the service of a community member and a Cloverleaf employee.  Community members and employees are invited to nominate individuals for the award utilizing the following criteria:

• Number of years of service
• Type of service
• Academic involvement
• Extracurricular involvement
• Specific incidents which make a candidate worthy for the award

A committee of individuals representing a cross section of the Cloverleaf employees and community members will be assembled to review the applications and determine an employee and community member to receive the award.
