The survey consists of multiple choice (select only 1 response), checkboxes (select multiple responses), and short answer (fill in the blank) questions. There are four sections of the survey. To move from one section to another, use the "Next" button at the bottom of each section. Once complete, use the "Submit" button at the bottom of Section 4.
We recommend completing the survey in one sitting - or at least keeping the survey open on your computer until done. Exiting the survey without submitting it will result in your answers not being recorded. While not recommended, you can submit a partially completed survey using the “Submit” button at the end of the survey. Then, use the "edit your response" link that will appear in the browser to go back and complete the survey.
Households with more than one congregant, can use the same link that was emailed to you (household). However, only one person at a time can complete the survey.
While this survey asks for your email address, it is only used for validation purposes to ensure non-invited people don't fill out the survey. Responses are anonymized. We will aggregate multiple choice responses to get statistics and summarize fill-in-the-blank responses. We will not be able to trace responses to you.
