IN-PERSON PROTOCOLS:
To attend an in person event, you must fill out the screening questionnaire (in addition to this form). You must also bring your own prayer rug for any events taking place in the masjid. There will be a thermometers available for temperature screening upon entering. You must maintain 6 ft of social distance at all times. Masks are required at all times.
