Memorial Wishes Plan

Unitarian Universalism honors each life as precious and unique, emphasizing the way we live rather than the specifics of our beliefs. Memorial and Celebration of Life services are opportunities to lift up the values and life choices of the individual, providing comfort and inspiration to those left behind. 

You may have already been reflecting on your lived experiences and what you have found meaningful. You probably have things you hope to be remembered for. It’s a good idea to share those insights with your family members and your minister.  

Spend some time with this form so that your wishes will be available when needed. Take as much space as you need for each answer. You can attach documents, links, and photos with explanations. If you’re ready to fill out some answers but not others, that’s okay! You can always ask to update this form or submit a new one at a later date.  If you want to do a life review, but would value support in that process, please contact the minister or a member of the Pastoral Care Team for that support. 

Paper copies can be mailed or dropped off to the office. If you are using the online form, you can send a copy to the minister by hitting the submit button at the end of this document. These forms will be confidentially filed in the church office. Consider giving copies to those closest to you.
