Spring 2026 Class Change Request Form
Only PHS students will be able to complete this form while signed into their student Gmail account.  Please know that the master schedule was built based on student course requests last Spring. As a result, opportunities for student schedule changes are limited.
  • PLEASE DO NOT SUBMIT MULTIPLE REQUESTS. Doing so will delay the processing of requested schedule changes, thanks!
  • Please be patient. It may take several days to review your request. 
  • Continue following your current schedule until you receive an updated copy from the counselors. 
**ALL SCHEDULE CHANGE REQUEST FORMS MUST BE SUBMITTED NO LATER THAN 3:15 on Wednesday, December 10th.**

I understand that that schedule change requests will only be considered for valid extenuating circumstances and only if the change can be implemented within the constraints of the master schedule. Changes will not be made based solely on preferred block, lunch period, or teacher.
