Custom Gift Basket Order Request
The Village Apothecary is excited to help take the stress of unforgettable gifting off your plate with our custom gift basket service! From the holiday season, to baby showers, housewarming gifts and everything in between, we're here to help!

To get started, please complete the form below including your name, number, budget, desired pick up date, and details about the recipient/desired inclusions.

Baskets can be picked up at 36 King St E in Millbrook during store hours:

Tues- Thurs:  10am - 6pm
Friday: 10am - 5pm
Saturday: 10am - 4 pm

To allow our staff time to put together the baskets, we require 72 hours notice for all custom gift basket orders. 

Once your order form is received and products have been selected, a member of our team will reach out for a credit card number to process payment.

Please note that we charge a $20 non-refundable fee for all custom gift baskets. Please include this within your budget provided.
