NORTH SHORE SUP, LLC AND ALL ITS’ ENTITIES & PROGRAMS PARTICIPATION AGREEMENT AND GENERAL RELEASE OF CLAIMS

WATERSPORTS CAN BE DANGEROUS, WE REQUIRE ALL NORTH SHORE SUP LLC AND ALL ITS’ ENTITIES AND PROGRAMS RELATED TO NORTH SHORE SUP LLC (“NSS”) PARTICIPANTS TO ASSUME ALL RISKS BY SUBMITTING THIS PARTICIPATION AGREEMENT AND GENERAL RELEASE (“AGREEMENT”). THIS AGREEMENT AFFECTS YOUR LEGAL RIGHTS.

In consideration of participating in any of the activities outlined below, and for other good and valuable consideration, I hereby agree to release and discharge from liability North Shore SUP LLC ("NSS"), as well as its owners, directors, officers, employees, agents, volunteers, participants, and all other persons or entities acting for them (hereinafter collectively referred to as "releasees"), on behalf of myself and my children, parents, heirs, assigns, personal representative and estate, for the entirety of the 2020 Season, and also agree as follows:

Only customers aged 18 or above may enter into this agreement. Customers under the age of 18 must have a parent or legal guardian complete this form for them.
