Patriot Shooting Camp is schedule for:

Week 1: League Play ONLY - June 29 - July 3

Week 2: Shooting Camp - July 6 - July 10

Week 3: Shooting Camp - July 12 - July 17

Week 4: Shooting Camp - July 20 - July 24

All Sessions are 9 AM—12 Noon. Cost $225

Optional Add On Afternoon League Play Session (12:30PM - 2PM). Cost $150

My child has permission to attend the Patriots Shooting Camp. I certify that within the past year, they have had a physical examination and that now they are physically able to participate in basketball camp activities without restriction. In the event of an illness or injury, I give consent for medical treatment, and permission to attending physician to hospitalize, secure proper treatment, and order injections, anesthesia, or surgery. I will be responsible for any medical or other charges in connection with my child's attendance in camp.

I acknowledge that at Patriots Shooting Camp, my child will participate in a sport that my involve among other things, physical contact of the body with other persons or objects, including the ground and they may incur a risk of injury. I specifically waive and give up and release Patriots Shooting Camp and staff from liability for any claim for damages which I or my child may have for injuries or illness that they may sustain at camp.

Permissions *

I grant permission for my child to walk to lunch with Patriots Shooting Camp Staff on the North side of Franklin Avenue to Bagel Nosh, Franklin Lakes Pizza, or Subway. Students are not allowed to cross the street.

I grant permission for my child to be photographed while participating in Patriots Shooting Camp. It is my understanding that the photographs will be used for promotional purposes only
