FRCO Participant/Alumni Survey

Thank you for your participation in the FRCO Participant/Alumni Survey.  As we embark upon the development of a 3-year Strategic Plan for FRCO, the input of our current and former program participants is essential to establishing our Strategic Priorities and Goals.  Your answers will help us understand the ways in which we can improve and grow in the future. 

This is a confidential survey. Your completed questionnaire will be returned directly to Executive Service Corps- Florida, the nonprofit consulting organization that is assisting us to develop our Strategic Plan.

Directions:  There are 10 questions to the survey.  You do not have to answer all of them for your input to be counted.  Most ask you to pick from a list of options.  In some cases, you have the option to provide additional comments. You will likely be able to complete the survey in 10 minutes or less.

It is important that you hit the SUBMIT BUTTON at the end of the survey in order for your responses to be included.
