In order for your application to be complete you must:1) Be a junior at Sequoia2) Be a member of the PTSA3) Submit this application form with the essays4) Upload a PDF of your transcript (follow directions below)

The essays are very important in highlighting your achievements and your personal story. Essays will be judged by spelling, formatting and grammar, as well as content. Please make sure you ask at least one adult staff member to review your essay (ie. English/History/AVID teacher, counselor, Ms. Ignaitis) for grammar and content.
Your name and any contact information will be redacted from all parts of your application before it goes to the PTSA selection committee. Please do not identify yourself in the essays. Your information will be held in confidence unless student safety is a concern.
