Hello! This form is for individuals within Sierra Club Florida, or whom are engaged with Sierra Club Florida, that are experiencing issues of discrimination, inequity, intolerance, or imbalanced power dynamics within this work.

That you have been brought here is unfortunate, but know that there are many volunteers and staff members working day and night to ensure this form one day becomes obsolete. It will take a lot of work! We are in it for the long haul.

Please complete the below form as much possible. You're welcome to leave parts blank in order to remain anonymous, but if enough information is not provided for an EIJ Committee member to take some sort of action, we will be unable to respond appropriately.

If you are a staff member, please follow your administrative/HR guidelines. We are still here to support you in whatever way is appropriate.
