Student Home Testing Result Recording Form
Please note that consent is given by opening the home test kit for use at home.

Having completed a test you should use this form to report the result to the Academy. You must also report the result to the NHS using the website www.gov.uk/report-covid19-result or by telephone on 119.

Action required if you test positive for COVID-19 during a Lateral Flow Device home test.
If you have recorded a positive test result the person who is positive and the whole household must now begin self isolation and you should book a PCR test in order to confirm the LFD result. The whole household must isolate until the PCR result is confirmed. If this is negative and there are no symptoms you may return to school/work. If the PCR test is also positive the whole household should continue to self isolate following Government guidelines and the advice given to you. You must also contact the Academy by telephone to let us know that your child has tested positive. Please confirm that you understand these instructions.

A PCR test can be booked at https//www.gov.uk/get-coronavirus-test or by contacting the NHS by telephone on 119. You should report the PCR result to the Academy on receipt. Please confirm that you understand the need to take this action.
