New Private Lesson Student Questionnaire
Please complete this questionnaire to be added to our prospective student list. Please fill out one questionnaire for each student, and be as specific and detailed as possible. The information you provide will help us match you with a teacher. Once lessons are confirmed, we will provide a copy to your teacher to help him/her understand your needs and plan your lessons.

Private lesson enrollment begins approximately one month prior to the start of the semester and continues after the semester has begun.

Please begin by reading these important CSMA policies:

2018-19 TUITION
All tuition at CSMA is charged by the semester. Each 19-week semester includes 18 lessons and Workshop Week. Financial aid may be available to families who qualify: http://arts4all.org/music-school/policies-forms/financial-aid

Tuition is as follows:
For a 30 minute lesson each week, tuition for the 19 week semester is $893
For a 45 minute lesson each week, tuition for the 19 week semester is $1292
For a 60 minute lesson each week, tuition for the 19 week semester is $1729

WORKSHOP WEEK
The cost of Workshop Week is included in your tuition, and is the same as the cost of one private lesson. All private lesson students may take as many or as few workshops during workshop week as they like at no extra charge.

MAKE UP POLICY
There is no credit, reimbursement, or refund for missed lessons due to a student’s absence and teachers are not required to make up a missed lesson due to a student’s absence. Some of our teachers strictly adhere to CSMA’s make up policy, and some teachers follow a slightly more lenient individual studio policy when they are able to. Any makeups or rescheduled lessons are completely at the discretion of the individual teacher and are never guaranteed.

WITHDRAWAL POLICY
CSMA does not offer trial lessons. However, we do have a very simple withdrawal policy. All students must register for the entire semester. You then have up until 3 days after the second lesson to decide if you would like to change teachers or withdraw. Please let us know in writing no later than three days after the second lesson and we will either work to find you a different teacher, or withdraw you from lessons and refund the cost of all unused lessons. After this “trial period” no refunds or teacher changes are granted.
