Mary Frank Early Learning Academy Registration Form
Registration for the Mary Frank Early Learning Academy will  be open to PHM residents ONLY  beginning January 16, 2024.  Beginning February 7, 2024,  non resident families may begin registering for any remaining spots available.

Please complete this form to begin the registration process for your child to attend the Early Learning Academy (ELA) located at Mary Frank Elementary.  Please note the following:

- The child must be age 4 before September 1, 2024.
- There is a $60 non-refundable registration fee that must be received before your child can be registered.
- There is a $35 a day fee for the Academy.
- The program operates 5 days a week; 8:00a.m. - 2:30p.m.
 
After submitting this form, you will need to email the following documentation to kforsey@phm.k12.in.us.

-  Original Birth Certificate
-  Two proofs of residency (current lease/rental/mortgage statement/bill and a current utility bill or property tax statement)
-  Immunization Records
-  Any legal documents related to child custody, restrictions, or restraining orders

We will register your child upon receipt of the listed documentation and the $60 non-refundable registration fee.  We will be in contact with you regarding the next steps in the registration process.
