ACRC Expedition Equipment Booking Request & Loan Agreement
The Australian Speleological Federation makes this equipment available through the Australian Cave Rescue Commission to ASF members.
This form should be filled out by the ASF member responsible for borrowing the equipment as it includes an agreement.

Usage agreement
Note that you are agreeing to return the borrowed equipment in good order to the ASF and agree to make good any damage.
If borrowing the satphone, you are also agreeing to pay all call charges incurred during the time you have the phone.
