I understand that Blue River Valley Schools require that students ride the bus to and from all athletic events and a departure from this requirement (if approved) will release Blue River Valley from all liability for any adverse results that may occur. I agree to release Blue River Valley and its employees and officers from all liability with reference to the above stated transportation.
This form must be approved by the Athletic Director or Principal. Assume that this request is approved automatically unless otherwise communicated to you through email from administration.
