MHS Chromebook Services Work Request Form
If you are having issues with your account, DO NOT fill out this form, but use your teacher's phone to call the RCS Help Desk (Ext. 72700).

This form is used to collect your information when you have a problem with the functioning of Chromebook or charger. You must complete all of the questions. Please do not come to the Media Center with your device unless you have been called as Ms. Wotring may be with a class or unavailable to assist you.
