This form must be completed in order for your program format change request to be considered. 

If you are requesting for a change to take effect in Fall 2024, please note that it is too late to request a field placement for Fall 2024 and the earliest you could begin a new field sequence would be Spring 2025.

If you are considering a change from the on-campus format to the online format, please keep in mind:

*The online format has more autonomous student-directed learning.

*There are differences in tuition costs.  Please see the tuition calculator to better understand those differences. https://sfs.vcu.edu/tuition-and-fees/estimate-your-tuition-and-fees/

*The online format is writing intensive.  In most courses, discussion boards are a regular feature.
