Jam Camp 2018 Registration
Registration opens April 1, 2018
Registration will close June 16, 2018

Camp schedules will be sent to the teachers after registration closes.

Teachers must have a valid e-mail address to participate. Teachers without an e-mail address should contact the registrar before registering.

If you are registering more than 12 students, please submit multiple registration forms. You will still submit one check for the total registration fees.

Audition videos should be attached to this form unless longer than 1 GB.
If longer than one GB then please email to: liz.piano2@gmail.com

Please proofread carefully! List all names exactly as you want them to appear in the recital program.
