Grade 3-6 Aquatic Water-Based Activity Consent Form 2019
Dear Parents,
Parental/guardian consent is required to enable students from Grades 3 to 6 to participate in the following aquatic activities:
- Swimming Trials (Tuesday 12th February) at the Clarence Aquatic Centre.
- Corpus Christi Swimming Carnival (Friday 1st March) at the Clarence Aquatic Centre.
- Southern Catholic School Swimming Carnival (Friday 5th April) at the Hobart Aquatic Centre.

The consent form is the Google Form below and must be submitted by 8.30am Tuesday 12th February 2019. Submitting this Google Form provides consent for each and all of these activities. If consent is not provided students will be unable to participate. The cost of these activities is included in Tuition Levies. Information and further details regarding the nature of the proposed activities have been provided in the emailed letter to Grade 3-6 families on Wednesday 6th February.

Your child in Grade 3 to 6 will be participating in each of the Corpus Christi Catholic School Swimming Trials and Carnival at the Clarence Aquatic Centre and, if selected to represent the School, will also be participating in the Catholic Primary Schools Swimming Carnival at the Hobart Aquatic Centre. In relation to the above mentioned aquatic activities please indicate the ability level of your child. *
I hereby give permission for my child to participate in the Corpus Christi Catholic School Swimming Trials at the Clarence Aquatic Centre on Tuesday, 12 February 2019; the Corpus Christi Catholic School Swimming Carnival at the Clarence Aquatic Centre on Friday, 1 March 2019; and, if selected, the Catholic Primary Schools Swimming Carnival at the Hobart Aquatic Centre on Friday, 5 April 2019. *
I understand that this will involve water-based activities/swimming and that persons with appropriate first aid, CPR and rescue qualifications will be present. *
In the event of accident or illness when it is impracticable or impossible to communicate with me, I understand that the teachers at the venue will arrange for my child to receive such medical or surgical treatment as may be deemed necessary, and agree to reimburse the school for any costs involved. *
I confirm that as requested, I have discussed with my son/daughter the need for them to listen attentively to information given on the day, to follow instructions and to not enter toilet/changerooms without prior permission. *
