Enrollment Packet
Please be aware that you must also submit in person the following forms:

☐ Image Release Form
☐ Counseling Form
☐ From your Doctor:
- DHS Medical Form (or waiver if applicable)
- Prescription Medication Form (if applicable)
☐ Copy of birth certificate
☐ Residency verification (2 forms)
May include utility bills, government card (driver’s license, voter ID, Medicaid, public aid), court documents, stamped USPS change of address form
☐ Proof of guardianship/custody
- May include court order, tax return with child as dependent, health insurance coverage, public aid documents
☐ Final report card (or most recent report card if final is not available by due date)

(Only if applicable)
☐ Individualized Education Plan (IEP) or 504 Plan
☐ CPS Algebra Exam eligibility letter
☐ Transcript or proof of awarded high school credit

PLEASE NOTE: Your child’s enrollment will not be processed if any of the required documents are missing, and the packet will be returned to you for completion. Failure to return these forms during Family Check-In, July 24-25, will risk your child’s admission being revoked.
