Bear River Health Department - Food Service Phased Guidelines Self Assessment
The Bear River Health Department encourages businesses to complete this self-assessment. This assessment is intended to be used as a planning tool for restaurants and food service businesses to use in preparing to move from a high to moderate COVID-19 risk category. The questions in this form come from Governor Herbert's "Utah Leads Together 2.0 Plan - Phased Guidelines for the General Public and Businesses - Version 4". These phased-guidelines allow for dine-in services with extreme precaution, following strict guidelines around physical distancing and staff monitoring. The guidelines also address takeout, curbside pickup and delivery options, which are encouraged.
