Waiver and Release Form for Garden Landscaping Project Consent Form

In consideration of the acceptance of my application for the historic garden landscaping community service program at the Ashbel Woodward Museum, I hereby waive, release, and discharge any and all claims for damages for personal injury, property damages which may occur during or after to myself and or my child as a result of their participation in said program. This release is intended to discharge in advance the Town of Franklin, its officials, officers, employees, volunteers, and agents from all liability, including any real or perceived negligence on the part of the persons mentioned above. It is understood that some recreational activities involve an element of risk and knowing those risks, I hereby acknowledge and assume those risks. It is further understood and agreed that this waiver, release, and assumption of risk is to be binding, to include my heirs and assignees.

Consent for Treatment: In case of an emergency, I hereby give my consent to have myself and or child to be treated for any injury or sudden illness while participating in the above activity.  It is understood that the Town of Franklin will not provide medical insurance for such treatment, and that the cost thereof will be at my expense. I have read and understood this liability release and parental consent form and agree to all of its terms and conditions.
