You only need to complete this questionnaire if you wish to take up the offer of key worker provision!

The Key Worker provision is full time provision for all 5 days from Monday 4th until Friday 12th February 2021. Students who access this provision will be expected to attend in full uniform bringing their school bag and usual equipment with them. They will be working in line with their normal timetable throughout each day, starting and finishing in line with their current school day using the normal year group entrances.

Children classed by the Government as vulnerable are expected to attend school full time from Monday 4th January to Friday 12th February 2021 and will be contacted individually by staff from the Academy. See link below for the definition of vulnerable children:-
