Survey Eligibility Statement

This survey pertains to your most recent pregnancy. To take part, you must meet the following criteria:

  • Be between 18 and 45 years of age

  • Have been pregnant within the last two years

  • Have used healthcare services in the Quad Cities for your pregnancy-related care, and/or gave birth in the Quad Cities.

Student researchers and/or other investigator names: Americus Mahatshahi, Selgelia Okine, Paige Rogde and Peyton Heisch                                                                                             Research Project Title: Socioeconomic Influences on Maternal and Infant Health: Insights from Lived Experiences in the Quad Cities

Purpose and Description of the Research Project:

This research aims to explore the barriers faced by women in accessing quality maternal healthcare in the Quad Cities. By conducting surveys, we will capture the lived experiences of women, focusing on how socioeconomic factors influence their healthcare journeys and outcomes.

Description of Participant Involvement:

Participants will engage in surveys lasting approximately 10-20  minutes. During these surveys, you will be asked questions regarding your experiences with maternal healthcare. Your participation is entirely voluntary, you may choose to withdraw at any time, and refusal to participate will involve no penalty or loss of benefits to the participant.

Talking about sensitive topics could potentially involve substantial risks. There may also be benefits, including contributing valuable insights that could improve maternal healthcare in the community. Your responses will be used for research purposes. All information collected will remain confidential and securely stored. Results will be reported anonymously to protect your identity, ensuring that no individual participant can be identified. We do expect to write a paper about this data that we will share with maternal health care professionals in our area, as well as the CACHW’s partners the Low Birth Weight TaskForce and The Healthy Pregnancy Coalition. After this paper is completed we will destroy all data. This data will not be used in other research.

This research has been approved by the Augustana Institutional Review Board, which can be reached at IRB@augustana.edu. Contact this email for answers to pertinent questions about the research and research participants' rights, and whom to contact in the event of a research-related injury to the participant

Consent Agreement:

By signing below, you consent to participate in this research study. You acknowledge the following:

  • You must be at least 18 years old to participate.

  • Your participation is voluntary, and you can withdraw at any time prior to study completion without any consequences.

  • Information shared during the survey will be used for this research only and will not be used in future studies.

  • All information will be kept confidential and secure.

  • Results will be reported without identifying individual participants. Any statistical data will be aggregated to minimize the risk of identification.

  • Identifiable information will be removed from your responses, or responses will be linked only to participant numbers. Your name and the number will never be stored together.

  • All research materials, including survey forms and recordings, will be destroyed within five years after the completion of the study.
