PLEASE BE ADVISED THAT IN THE EVENT THAT THIS CALL IS FOR A LOCKOUT OF ANY SORT (AUTO, HOME, ETC.) CHAVERIM REQUIRES PROPER ID UPON ARRIVAL, AND PROOF OF OWNERSHIP MUST BE SHOWN UPON OPENING OF VEHICLE/HOME. IF PROPER ID AND PROOF OF OWNERSHIP CAN NOT BE PROVIDED, CAR/HOME WILL BE RELOCKED AND LOCAL POLICE DEPT. NOTIFIED.

1.  GENERAL RELEASE: I hereby release and hold CHAVERIM harmless from all claims that I may have arising from services rendered by CHAVEIRIM. I am providing this Release and Waiver of liability to CHAVERIM so that I may receive service from a CHAVERIM volunteer.

2.  ASSUMPTION OF RISKS: I hereby expressly and specifically assume any and all risk of property damage resulting from services rendered by CHAVERIM. I assume the risks: I understand that CHAVEIRIM services may come to personal property damage. While CHAVERIM takes all steps it can to avoid property damage, I agree that I am responsible and agree for CHAVERIM to perform the service.
Once you sign, you are saying that you understand the risk involved and accept all of the consequences. If you do not understand this, please ask the volunteer any questions you have before signing this Release.

3.  INSURANCE: YOU ARE EXPECTED TO HAVE YOUR OWN INSURANCE. You should understand that CHAVERIM may not carry insurance to cover losses that may occur while a Volunteer is providing service. I am not relying upon CHAVERIM to maintain insurance coverage should any repair be necessary.

Waiver and Release: Your Release is complete and forever. It forever discharges and holds harmless CHAVERIM, its successors and assigns, from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise resulting from services rendered by CHAVERIM.

Your Release is complete: It discharges CHAVERIM from any liability or claim that you may have against CHAVERIM with respect to any bodily injury, personal injury, illness, death or property damage that may result from CHAVERIM services rendered.
Such claims may be for personal injuries of any type whether arising in tort or in equity or in any other form. Your Release is complete whether your claim is due to the negligence of CHAVERIM or is based on any other legal or other theory whatsoever. You also understand that CHAVERIM does not assume any responsibility for or obligation to provide financial assistance or other assistance, including but not limited to medical, health or disability insurance, in the event of injury or illness.

Scope of this release: This Release is intended to be as broad and inclusive as permitted by the laws of the State of Texas. This Release shall be governed by and interpreted in accordance with the laws of the State of Texas. You agree that in the event that any court of competent jurisdiction shall hold any clause or provision of this Release invalid, the invalidity of such clause or provision shall not otherwise affect the remaining clauses or provisions of the ‘Release,’ which shall continue to be enforceable

Assumption of the Risk: I understand that services rendered by CHAVERIM may involve damage to my personal property by a volunteer’s activities. These activities (including, but not limited to, unlocking vehicle doors, picking locks to residence or place of call, changing a flat tire and any necessary action involved in the change, boosting a car battery, etc.) may inherently damage and may cost money and time to repair. I accept this risk as if CHAVERIM had not taken any steps to assist me today.
