This mid-term evaluation is meant to provide feedback on your experience with the intern thus far. This evaluation will take approximately 10-15 minutes to complete, and will be shared to the student intern's department faculty sponsor via email provided below. A copy of your responses will be emailed to you in the event you need to re-send, or keep for your records.
