If you are a resident of the 45th Assembly District and are interested in receiving a backpack for a school-aged child, please fill out this form.

RSVP is required. Backpacks are limited to two per family. Must be a resident within the 45th Assembly District.

Although we have reached capacity with our distribution, if there are additional supplies or if an individual does not retrieve their backpack on the date of pickup, we will contact you to pick up items from our office at a date convenient to you next week.
