Please submit this form if you plan to enter into a contract to procure a web-based tool or mobile app for use in University services, programs, or activities.  

You do not need to submit this form if:

• The web-based tool or mobile app will be purchased through a Request for Proposal (RFP) process.  In this case, the accessibility review will be conducted as part of the RFP process.

• The contract for purchase of the web-based tool or mobile app will contain an unaltered accessibility provision that requires compliance with the Policy.  In this case, you may choose to submit this request for an accessibility review of the product.

• the web-based tool or mobile app is covered by one of the exemptions described in this Administrative Procedure:  Exemptions from Accessibility Standard Requirements for Certain Digital Content and Information Technology.  In this case, you may choose to submit this request for an accessibility review of the product, but this is not required.

Where possible, the Office for Digital Accessibility strives to complete accessibility assessments within two weeks of receipt of this form. However, due to the volume of requests currently in the queue, completed reviews may take much longer.
