As the parent or legal guardian of this child, I hereby release the 2020 DSHS Speech and Debate Digital Summer Boot Camp, the 2020 DSHS Speech and Debate (DSHS SAD) team, its captains, and its members, and any person(s) designated by the captains to serve in the time period mentioned above, from any financial obligation(s) resulting from any accident or injury that may occur to my child during the time period mentioned above. *
By typing my name and digitally signing below, I, as Parent/Legal Guardian of the student listed on this form, hereby knowingly assign to the 2020 DSHS SAD captains and boot camp lecturers, the right to record my child's voice and likeness for use in media productions. The 2020 DSHS SAD captains have absolute right and permission to copyright and/or publish photographic portraits or pictures or videos of my child, in which they may be included in whole or part, or composite or distorted in character form in conjunctions with their own or a fictitious name or reproductions thereof in color or otherwise, made through any media at the captains’ disposal for art, advertising, trade or any other lawful purpose whatsoever. I hereby waive any right that I may have to inspect and/or approve the finished product or the advertising copy that may be used. I hereby release, discharge, and agree to save the 2020 DSHS SAD captains and all affiliates, from any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form, whether intentional or otherwise, that may occur or be produced in the taking of said pictures or videos, or in any processing tending towards the completion of the finished product. *
