The student named above will not be riding home on the bus with the JCHS Band. By entering my name below, I am releasing JCHS and the JCHS Band from any liability. I understand that my student will not be allowed to leave the supervision of the JCHS Band staff without being physically present to check-out my student. I also understand that before check-out is approved, I must physically initial the bus list next to my student's name to approve their release. Upon check-out, my student is responsible to take their uniform jacket, hat, bibbers and gauntlets with them and return them on the next school day following the event. *
