Thank you for participating in this questionnaire and for your ongoing support of our ministry. The feedback you provide here will help to inform the Board in creating protocols for re-opening our facility after the local government has lifted its ban on public gatherings. We will not share your name and/or specifics with others, so please speak truthfully and frankly.
When our local government lifts its ban or guidance against churches meeting, which best describes your attitude about physically returning to our spiritual center? Select one:
