Welcome to the 2019-2020 Registration for St. Anthony of Padua School! After completing this form online, parents/guardians are still required to attend their assigned registration night in February and sign the official tuition contract with Mrs. Grey in person. Parents and students must read a copy of the Parent/Student Handbook online at www.stanthonygardena.org, and sign a hard copy stating that they read and understood it.

FACTS payments are due on the 5th or 20th of the month. *
Tuition is to be paid through FACTS Tuition Management Service ($50 annual fees apply)
If I need to apply for tuition assistance, I will apply first through CEF, before applying for financial aid through the school. *
This contract agreement may change from year to year, and must be renewed and signed each year. If I apply for CEF and do NOT qualify because my income exceeds the guidelines, I may apply online through PSAS (a $30 fee) to be considered for school financial aid.

Final grades and report cards will not be released until all tuition fees are paid. *
8th graders will not be able to walk at graduation or attend 8th grade festivities without paying all graduation fees and tuition in full.

Fundraising/Family Financial Commitment
To keep tuition costs low, in comparison to other Catholic schools in the area, all families have a $500 fundraising/financial commitment (per family). *
This can be rolled into your FACTS tuition payments to avoid fundraising for this reason, or be paid in advance.

Option 1: Do Not give me any fundraising items. I will Pay in full upfront or roll into months FACTS payments. Option 2: Package selection which includes school suggested fundraisers (Candy, Hawaiian Opportunity drawing, Script).
Any fundraising fees not accumulated and outstanding fines will be billed at the end of May. Invoices will be sent out. You can always pay out $500 in advance or in May or roll it into your yearly FACTS tuition to avoid fundraising. There are no roll overs to the following year's tuition and fees. *

I understand that I must serve on at least one PLC committee and work 25 hours of volunteer service. This work is valued at $10 an hour. If not fulfilled, I will be charged $250 for non-participation. Volunteers can work more than 25 hours a year, but there will be not rollovers for the next year. *
