Vizslamentés UK Rescue Trust Application Form
IMPORTANT: please read before completing the form.............

Adopting a rescue dog, that has probably been abused and mistreated, can bring different challenges (but also special rewards) to obtaining a young dog from a breeder. The dogs may well take time to adjust to their new home, and often will need basic training, sometimes house training, as you would a puppy.

These rescue dogs are very special, which is why Vizslamentés takes such care to match the needs of the dog as closely as possible to the ideal and forever home.

So please forgive us if this questionnaire seems lengthy, or some of the questions a little intrusive. Please complete all sections with as much detail as you can manage to help us to build a picture of the home you are offering to a res cue from Vizslamentés. We will ask applicants who do not complete their application in sufficient detail to submit again.

All questions are compulsory and the entry boxes will remain outlined in red if not completed and the form will not be able to be submitted unless complete. You will receive an on screen submission (please don't close your browser until you have seen the message).

After submission the members of the Vizslamentés team will review your form in detail and we will advise you of the outcome afterwards. If we decide to take your application to a homecheck then we will locate someone from our network of volunteers. Please bear with us during this process, we do our best to progress applications in a timely fashion.

Thank you for your interest in adopting from Vizslamentés, we look forward to receiving your form.

Vizslamentés neutering policy - the majority of our adoptive dogs are not neutered before they travel to the UK. *
In the case of a puppy adoption, the adopter agrees to neuter or spay the puppy no earlier than 18 months of age. In addition, if the puppy is a female, she should have had a minimum of one season, but preferably two and the spay operation should be carried out at least 3 months after the end of the season. All adult dogs which are already over the appropriate neutering age are to be neutered after a settling in period of not less than 3 months.

Describe for us why you wish to adopt this kind of breed, what you understand of their needs and nature and how your lifestyle suits life with one, or more!
Training ethos *
Describe for us how you plan to train your dog, please help us to understand your approach to dog training either from experience (if you have a dog already, or have owned before) or from research and reading (if you are new to dog ownership) Please share with us your aspirations from a training perspective and how you intend to achieve them. There may be authors or TV personalities that you have an affinity with which will help us understand your approach.
Behavioural/adjustment issues with your dog *
All rescue dogs, no matter what the age, gender or history will have a period of adjustment on arrival at their new home. All dogs are thoroughly assessed by their foster home in Hungary and this information is used in helping us to match a dog to its forever home. Please describe how you would help your dog to settle to their new life in the UK, how you will train, classes and whether you plan to crate train (dogs in Hungary are not crate trained)
Rescue vs puppy from a breeder? *
Rescuing brings a completely different set of challenges to buying a puppy from a breeder. Please describe in as much detail as possible why you are choosing to rescue rather than buy a puppy.

Who would you contact if you needed to give up your dog? *

Your adoption timescales and form submission
Vizslamentés has, at any moment in time, many dogs in foster care in Hungary. All our dogs are fostered in family homes, and all the dogs need forever homes as soon as we can locate their perfect home. A dog in foster care long term blocks a space for a new dog needing to be rescued. We ask that you consider your own timescales for adoption and let us know by answering the 2 questions below.
We are ready to adopt now, no holidays booked or pending issues to delay the process *
Please choose yes or no below and if you aren't ready to adopt indicate your timescales in the next section.
We wish to start the adoption process but are not ready to adopt yet - please indicate the earliest date you are able to adopt from
Please let us know the earliest approximate date you could adopt from Vizslamentés. This helps us to plan and prioritise the next stage in the adoption process.
Anything you want to add? *
Please use this box to tell us anything not covered by the form that you think we should consider when reading your application. We have asked a lot of questions which help build a picture of your home but this is your chance to share anything with us that you'd like us to know beyond the formality of the application. We read all forms in detail and the more your share with us the more we can understand the kind of home you offer.
The adoption process with Vizslamentés *
Our adoption process and the adoption requirements are attached to the email you were sent with the link to this application form. There is a lot of important information included in those 2 documents and we ask you to read them very carefully before submitting this form. Please confirm you have read and understood the paperwork by answering yes/no in the box below. If you have any questions please put them in the final box and we will respond as soon as we are able.
Any questions?
The paperwork we share with you has lots of information but there will always be questions we haven't covered. Feel free to list them here and we will do our best to respond and answer them for you.
