CVM Teen Guide Volunteer Application
Teen Guides advance the Chippewa Valley Museum's mission to connect people to our community and inspire curiosity by collecting, preserving, and sharing our region's historical and cultural resources. Program participants share the museum's resources with the community by volunteering in the Ice Cream Parlor, Museum Store, or at the Admissions Desk. Teens also play an important role in special programs, including the annual July 4th Family Fun Fair and Kaleidoscope summer learning programs for younger children.

Please check each box below to confirm you plan to meet each of the program requirements:
Meet the age requirement (13-17 years old)
Submit complete application (this form + recommendations) by April 17, 2020
If accepted, attend ONE Tuesday evening orientation: Either May 5 or May 12 6:30-8p
If accepted, attend ONE 3-day training session: Either June 9-11 or June 16-18 10a-2:30p
Following training, complete 32 hours of service-learning (shifts are available 7days a week)
Have parent/guardian permission (a check here indicates you have necessary permission)
Secure necessary transportation for program participation
