Before completing this application, please make sure you meet the following requirements:

* Physical fence required with a few possible exceptions based on specific dogs.
* All current animals MUST be spayed/neutered and up-to-date on all vaccinations and heartworm preventives.
* A veterinary reference is required.
* A home visit is required
* Operation Education does not support the use of tie-outs and reserves the right to deny any foster application and re-claim any animal if the foster is found to be in violation of this policy

* I understand and agree that I am volunteering my time, home, and services and will not be compensated by Operation Education Animal Rescue by any means. I also understand that I may be removed from this position according to the organization's rules and procedures at any time.

Any major decisions regarding the foster animal(s) in my care including but not limited to: food change, veterinarian care, moving, introducing another animal into the home, etc. should be approved by Operation Education prior to implementation.

Periodic home visits to our foster homes & animals are an integral part of this organization. If you are approved as a foster home and an animal is placed with you, representatives will schedule these periodic visits in conjunction with your schedule, to the best of their ability. If having a representative visit your home will be an issue, please make us aware before taking an animal.
