  • Anyone who volunteers with West Tuality Habitat for Humanity must fill out this form once in each calendar year that they volunteer. 
  • All information is kept within our organization. 
  • You will be added to our quarterly email newsletter list but you can unsubscribe anytime after you receive the first newsletter. 
  • If we text you on the phone number you provide, it will be only urgent messages, such as the cancellation of that day's volunteer activity because of weather.
  • Volunteers under the age of 18 must fill out this waiver but then ALSO their parent needs to fill out a SEPERATE  waiver. An adult chaperone (does not have to be the parent) must accompany a youth volunteer during the day. Chaperones must ALSO fill out this waiver.
