3BC ADHD Parenting Class Interest Form
Thank you for your interest in the Parenting Your ADHD Child class!
To make sure the class is a good fit for what you’re looking for and for what we offer, we’ve laid out some of the specifics of the next round of class, below, and ask that you provide a couple bits of info.
Please note: this form is just a pre-screening.
If the class is a good fit for you and your family, once you’re ready to register, we will enter you into our electronic record system, Simple Practice (secure and encrypted online portal).
Please note: This class is designed for parents of a child with ADHD; however, a diagnosis of ADHD is not 100% necessary to join. Even without a diagnosis, this class will help if your child is struggling with ADHD-like behaviors.

Some people who participate in this class have children who have already received a clinical diagnosis of ADHD, some are in the process of pursuing an ADHD evaluation, and some have children who do not meet criteria for ADHD but are wanting to participate in the class to learn strategies for improving behavior and family relationships.

Please note, this class is not appropriate for parents of children who have received a diagnosis of Conduct Disorder.

Has your child received a formal diagnosis of ADHD?

Has your child received any other diagnoses other than ADHD (i.e., ODD, anxiety, depression, or any other clinical mental health, behavioral and/or learning disorder/difference
If so, what?
Some people who participate in this class have never attended a parenting class or read or learned about any particular parenting approach (which is OK!!) but it can help us to know if you have participated in one before and/or if you use a particular parenting approach with your children.
What parenting class(es), parenting books, or approaches have you participated in or use with your children? (i.e., Conscious Discipline, PBIS, Yelling, Time-Outs, Incentives, Spanking, etc)

The General Outline/Topics for the 8 weeks are as follows:

2. Paying Attention

3. Increasing Compliance and Independent Play

4. When Praise is not Enough

The structure/format of this class is a didactic model of teaching and coaching strategies. Sometimes Powerpoint & lecture will be used. At times, there may be questions or prompts posed to the class members and opportunities to role-play parenting scenarios or discuss common difficulties. This is NOT a group therapy offering but rather formatted to deliver you content in a live class learning environment. Across the 8 weeks, you will receive 21 handouts (in PDF form) related to content covered. Topics for the 8 weeks are listed below (though order may vary). Our goal would be that you would gain practical skills & tools toward creating more harmony and less chaos in your day-to-day parenting.

When you sign up, you're agreeing to pay for your spot in all 8 of the 90 minute classes. The cost is $65/class. If 2 parents (from the same family; divorce included) register, the cost is $100 (i.e., $50 each)/class. You can pay this weekly OR if you’d like to pay the full sum up front, we offer a discount of $50 off total cost if paid in full by first class (so total cost would be $470 vs $520 for 1 parent) OR $750 vs $800 for 2 parents).

In registering for the class, your spot would be reserved just for you, so you are agreeing to pay for all 8 classes, even if you miss one or drop out. We will not add folks to the class after the 2nd week. The material is progressive, building upon the previous week. The non-refundable deposit for registration is $65 ($100 for 2 parents).

Is this something you’re ready to commit to? And how many parents will be participating?
