Report Form for ESN Latvia's Code of Conduct

Code of Conduct is a set of rules outlining the norms, responsibilities, and proper practices for individuals within an organization. Examples of Code of Conduct violations include making discriminatory remarks, harassment, bullying, improper social media conduct, and intoxication during ESN-related activities.

This form should be used to notify that an incident has occurred which violates ESN Latvia's Code of Conduct. If an incident occurs during an ESN Latvia event, it should be reported to the event's point of contact (normally the Chairing Team). This form is for reporting online incidents, or for events where you do not feel safe reporting to the point of contact.

This form will be sent to the National Board member. Reports relating to incidents that occur at a local level will be forwarded to your Local Board to take further action unless you tell us not to.
