Social Skills Interest Form-Strong Foundations Behavior Services
Thank you for your interest in our Social skills classes!  We offer enrollment in social skills classes on a rolling basis, based on interest and therapist availability.  

Our current class times are Tuesdays or Thursdays 4-5pm, depending on age and current social skills.

The groups are subject to change based on age, current social skills, and therapist availability.  You will be contacted if any changes to the social skills class day/time occur. Please describe your child's current skills in the form below.  

It is highly recommended that your child attend every class for 10 weeks in order to cover all of the social skills topics.  The group rate is listed below and must be paid every week as your child attends.
