Social Skills Interest Form-Strong Foundations Behavior Services
Thank you for your interest in our Social skills classes!  We offer enrollment in social skills classes on a rolling basis, based on interest and therapist availability.  Once you sign up, you will be contacted to schedule your first class and receive information on times and days available.

The groups will be scheduled according to age, current social skills, and availability.  Please describe your child's current skills in the form below.  

It is highly recommended that your child attend every class for 10 weeks in order to cover all of the social skills topics.  The group rate is listed below and must be paid every week as your child attends.
