I hereby consent to and authorize the taking and capture in any form, including but not limited to photographs, film, video recordings, videotapes and all other visual and/or audio mediums, by SCMEC or anyone authorized by SCMEC, of my image, my voice and related images, conversations and sounds (the "Recordings") taken for the activity that you enter below.

I hereby willingly grant, without compensation, to SCMEC and its assigns, the irrevocable and unrestricted right, but not the obligation, to use and publish the Recordings in any way it desires, including for editorial trade, advertising and any other purpose, and in any manner and medium (i.e., print, radio, television, Internet, etc.), including the right to alter the same without restriction. I hereby release SCMEC and its assigns from all claims and liability relating to said Recordings.

I hereby agree that all Recordings and all related biographical materials that I have provided are owned by SCMEC.

I hereby waive any right of inspection or approval regarding my appearance in the Recordings or the uses to which they may be put. SCMEC may edit and/or include the Recordings or any portion thereof in such uses as are permitted above, at its sole discretion. I understand that SCMEC may, in its sole discretion, elect not to use my image.

There is no time limit on the validity of this release nor is there any geographic specification of where these materials may be distributed. I have been given a copy of this release form for my records.
I hereby acknowledge that I am 18 years of age or older, and have read and understand the terms of this release and willingly sign this release on my own behalf or on the behalf of the minor named above.
